OSHA Bloodborne Pathogens Training Test
(at your expense)
Any Bloodborne Pathogens test Is NOT a tattoo
or piercing license or certificate permitting you
to tattoo or pierce in your home (or anywhere else).
The Texas Dept Of State Health Services
Regulatory Licensing Unit is the ONLY facility that can
issue a tattoo or piercing license permitting you to
tattoo or pierce. (Located in Austin, Texas)
Please Note : A tattoo or piercing "certificate"
does NOT exist in the state of Texas permitting you
to tattoo or pierce in your home (or anywhere else).
We can NOT accept your tattoo or license (or certificate)
that was issued to you to use at a different location in
Texas (or) in any other state. In the state of Texas
you are required to obtain a tattoo or piercing license
(separate licenses), per location
(exact address on the license),
This does include a temporary license
(7 days per temporary license)
(exact address on the temporary license)
as is required from the Texas Health Dept.
You Are NOT required to obtain a tattoo or
piercing license in order to take a course
at our school. My Tattoo & Piercing License
for my school covers anyone that is tattooing
or piercing at the school location that is
listed on the license. This does NOT cover
any tattoo or piercing services outside of
the school location
This does include but is not limited to :
Your house, a temporary event or ANY place
other than the address that is listed
on the tattoo & piercing license

Criminal Background Check (at your expense)
Includes Any Terrorist Activity Background Test.
We require a worldwide & international criminal
& terrorist background check. This includes a
background check of every state & city that is
in the United States. This also does include many
countries that are NOT in the United States.
This must be successfully completed & I must have
all of the results before you can start a payment plan
or be able to start anything in our establishment.

We will need all of the original results
that no symptoms or risks exist.
We also need the original shot records
of all vaccinations that have been received.

must be kept current & done at your expense.

If you don't have the lifestyle
to be able to achieve
your full amount
of hours & days successfully,
don't enroll to be a student
in our school.

If you are late for a personal payment plan payment
There is a late fee charge.
Your late fee charge is 5% of the amount
of your personal payment plan payment.
If you are more than 30 days late
for a personal payment plan payment,
You are disqualified from your course
and from your personal payment plan.
You will get an additional 60 days
to pay all of the late fees that are owed
and to catch up to where you need to be
on your personal payment plan payments.
(This Option Is Only Allowed 4 Times)
If the additional 60 days has expired,
You don't owe us any late fees
or owe us anything towards your
original personal payment plan payment.
You must start over
on a brand new
personal payment plan.
Your new personal payment plan payments
will be towards a brand new course.
(If We Have Any Available)
You must register, rebuild a new schedule,
all of your tests & shots, ect, must be kept current.
